COMPUTERIZED THREE-PART DEDUCTION FORM (Called the B-I Form) illustrating personal data, beneficiaries, medical participation, group life, group disability, group dental, optional term, dependent term, family dental, vision, money fund,athletic club, flex spending, tax sheltered annuities selected. The form is specially designed for flexible benefit plans, tracking some premiums before tax and some premiums; after tax. Each form is completed on computer each year by personal interview in our local office, by same Representative each year. The form is verified and signed by employee and the Webb office and copy of form is kept by employee, webb office, and school administration office.
COMPLETE DATABASE is generated and maintained on a monthly basis and numerous forms and reports are created each month.
CHANGE REPORT is prepared
illustrating all employees terminating, adding or changing coverage or data. This change report will show the difference between previous month and current month. This report is of vital importance each month to the school administration as well as the Webb office.
CONSOLIDATED REPORT of all coverage for all employees, generating employee deduction totals as well as each company premium total. This report balances with all B-I forms each month and is given to the school administration office each month, requesting a deposit to a fringe benefit account for a grand total of premium to be remitted.
GROUP LIFE, OPTIONAL TERM, AND DEPENDENT TERM PREMIUM REPORT This report will work simultaneously with the next report matching premium with volume. All coverage is determined by age and salary. Therefore if age or salary changes in any given month, this report will automatically calculate new premium and volume to be remitted.
GROUP LIFE, OPTIONAL TERM, AND DEPENDENT TERM VOLUME REPORTThis report will convert the premium report figures into the volume amounts of insurance. The volume figures are the amounts that are shown on the company reports when remitting the premium each month. These two reports keep you in balance at all times, and are of vital importance.
GROUP DISABILITY REPORT illustrates employee name, salary, birth date, disability benefit for each employee based on 66% of annual salary. (Salary increases are generated once each year as well as changes from part time to full time) A company group form is generated from this report and keeps you in balance each month with salary and benefits.
DELTA DENTAL report illustrates participation in employee only, employee and spouse, employee and children, and family coverage. On the B-I form employee premium is shown separately since it is one of the compulsory coverages. The dependent premium is shown in a separate field. This report adds those back together making a checking device for the company billing received. (Cobra Premium is also remitted with this billing each month)
VISION INSURANCE REPORT illustrates participation in employee only, employee and spouse, employee and children and family coverage. This report is compared with the company billing received form the Vision Company. Changes are always noted and premium remitted. (Cobra Premium is also remitted with this billing each month)
CANCER REPORT illustrates employees participating and policy numbers and premium amounts. These policies can be kept when employment terminates. Comparison is made with company reports received and premium is remitted.
INDIVIDUAL 5 YR RENEWABLE TERM REPORT These reports are generated for various companies illustrating employee premium, policy numbers, date of birth, name, etc. These reports are compared to Company reports received and premium is remitted.
COBRA REPORTS are generated for dental and vision each month. Premium is received from direct billing process for 18 months for employees who choose to continue dental or vision insurance.
MISCELLANEOUS REPORTS are generated each month for census data requests on calculating premium rates. This helps us to continually shop for best rates and best companies. We can also generate medical reports that assist the school district in tracking medical premium. Our new product is the Athletic Club this year and this will also generate new reports for tracking.

FLEXIBLE BENEFITS PLANS OFFERED

STATE HEALTH INSURANCE - Webb & Associates is not affiliated in any way with the State Medical Plan but assists with the enrollment, tracking of premium or problem claims. The Webbs will continue to support the state medical program. Premium is deducted before tax.
